The end of the Sorpranos as Rod Sterling might have seen it

Anthony Sorprano and Carmela Sorprano. To happier times. Meadow gets knocked up by Sil. Tony finds out and goes straight to the hospital where Sil is recovering from a gun shot. He finds Sil lying almost dead in his hospital bed and pulls out all the plugs that are keeping him alive. Sil dies a slow death. Just then Paulie walks out of the bathroom and sees that Tony has murdered Sil. Tony is stunned, pulls out his gun and shoots Paulie in the chest three times. Tony runs out of the hospital covered in blood and tries to find Carmela to tell her what just happened and that Meadow is pregnant with Sil's baby.
In the meantime, Janice blames Bobby's death on Tony and holds Carmela hostage with a gun to her head while hysterically screaming Bobby's name into her ear.
On his way to find his wife and children, Tony stops off to get a hero because he is starved from killing his two best friends. When he gets to the deli counter to order, the guy making the hero looks at him funny and points to the last table in the corner. There is Phil and he is not alone. Tony quickly pulls out his 45 and blows Phil's head off and then shoots the four guys who are with him killing them all. He eats half the sandwich and throws the rest on the floor. Once Tony reaches the house where his family is hiding out, he sees Janice with the gun to Carm's head.
Dr. Melfi hears Tony and runs in from the next room. She had called Carmela on the phone and was there to warn her that Tony was a psychopath and might try to kill her next.
Meadow wakes up from a deep sleep and realizes what is going on. She grabs Janice's gun and shoots her between the eyes, saving her mother. AJ knocks Meadow down and grabs the gun trying to stop her from shooting. The gun goes off accidentally killing Meadow. At the sight of his sister lying there dead, AJ puts the gun into his mouth and blows the back of his head off and his brains splatter out all over his father's face. Tony falls to the ground horrified.
Carmela jumps up and starts screaming at the sight of her two dead children. Dr. Melfi runs over to comfort her out of instinct. Tony sees this as an attack on his beloved Carmela and shoots Dr. Melfi in the back. She drops to the floor with her mouth open dripping blood out of it and she screams.
Tony thinks she is still alive and shoots again and again. The bullets go straight through her and into Carmela's face and stomach killing her instantly. Tony walks out the door blinded by AJ's brain in his eyes and slips on a banana peal and breaks his neck and dies.
Alternate ending: Tony moves to Venice Beach, California and has his face reconstructed so he cannot be recognized. He joins the Gold's gym and gets into bodybuilding and competes in the over 40 class.
The End.

I don't think that I can top it, but I do have an idea of what might happen.
Tony realizes that he has to do it himself. No problem. He's an excellent killer. Probably the best in his, now decimated, crew. Tony finds out where Phil is hiding through an unlikely mole in Phil's own gang! Yes, Tony has his won spies who have been working for him all along, and don't like Phil and the threat that he poses to the NY mob.
Tony KILLS Phil!!!
Unfortunately, the FEDS also know where Phil has been hiding, and they arrest Tony. Tony uses his "get out of jail free card" that he received for giving the names of two middle easterners (who he suspected of being terrorists) to the FEDS. Tony is placed in the witness protection program, along with his family.
Paulie, who is only really loyal to himself, is mortified that Tony is cooperating with the FEDS. He takes over what is left of both families, NY & NJ, and becomes the de facto boss. Tony is at the TOP of Paulies hit list for being a RAT!
That's my guess. Of course, there will be plot lines relating to other Sopranos characters, as you pointed out in your version. AJ, Meadow, Carmela, Dr Malphi, etc will all fit in the story somewhere.
AJ kills himself and then he shoots himself and just as tony is dying, his shrink calls him to say she'll take him back...it's possible, right?
